RECOMMENDED ACTION: Appoint Brian Campbell to the Library Board of Trustees; Lisa Craig
to the Site Plan & Architectural Review Committee (SPARC) and
Jack Fiori to the San Joaquin County Mosquito & Vector Control
District; and re -post for vacancies on the Lodi Senior Citizens Commission,
BACKGROUND INFORMATION: The City Council directed the City Clerk to post for one vacancy on the
Library Board of Trustees on September 15, 2021. Council also
directed the City Clerk to post for two expiring terms on the Lodi Senior
Citizens Commission, one expiring term on SPARC and one expiring term on the San Joaquin County
Mosquito & Vector Control District on October 20, 2021.
The Mayor reviewed the applications, conducted interviews, and recommends that the City Council concur
with the appointments as detailed below.
No applications were received for the vacancies on the Lodi Senior Citizens Commission. It recommended
that the City Council direct the City Clerk to re -post for these vacancies until filled. Government Code
Section 54970 et seq. requires that the City Clerk post for vacancies to allow citizens interested in serving to
submit an application.

recommendation to appoint you to the Library Board of Trustees for the remainder of a three-year term
(expiring June 30, 2022).
Please take this moment to read the attached Oath of Office, sign and date the form, and return to the City
Clerk's Office at your earliest convenience.
A copy of this letter will be forwarded to the City liaison for this group who will contact you soon regarding your
responsibilities and the dates and times of upcoming meetings.
Also attached is the City of Lodi's Commissioner's Handbook. It has been prepared to contribute to your
general knowledge and understanding of public affairs and to aid in identifying the scope and parameters of
your duties and responsibilities. While participation on a City commission is a major responsibility, we hope
that it will prove to be a meaningful and rewarding experience for you. City service provides opportunities to
gain a greater understanding of the issues facing municipal government and to become actively involved in
resolving those issues in a manner that reflects the best interest of the community.
As explained in the Handbook, as an appointed member to a City commission, it is important to comply with
the following requirements in order to remain in good standing as a member:
All commissioners must file an Oath of Office form with the City Clerk's Office within 30 days of
appointment (or reappointment) and prior to participating in any commission meetings or activities.
All commissioners must comply with AB1234 ethics training specified by Government Code section
53235 within 30 days of assuming office and thereafter by December 31 of even -numbered years.
(EXCEPTION: Greater Lodi Area Youth Commissioners — Student Appointees)
Commissioners must comply with Fair Political Practices Commission (FPPC) regulations pertaining to
Conflict of interest — Form 700s and are required to file an Assuming and Leaving Office Statement
within 30 days of assuming or leaving office and annually thereafter by April 1. (This requirement
pertains to Library Board of Trustees, Lodi Arts Commission, Lodi Improvement Committee, Measure L
Citizens' Oversight Committee, Parks and Recreation Commission, Planning Commission, and
SPARC.)
Ethics training is provided free of charge online by the FPPC at httv./Iwww.fppc.ca.gov/learn/public-off icials-
and-emto ees-rules-/ethics-trainin .html. Please choose the "Local Officials" training. Once your training
session is completed, you must print the Certification of Completion and provide a copy to the City
Clerk's Office. Please note — your certificate must indicate that training has been for a minimum of two
hours.
It will be necessary for you to complete a Form 700 — Statement of Economic Interest Assuming Office
Statement no later than 30 days from the effective date of your appointment (due Friday, December 31,
2021). Please refer to the enclosed Resolution 2021-11 for disclosure category requirements regarding your
particular board, commission, or committee. The City of Lodi utilizes NetFile to electronically file Statement of
Economic Interest forms. Online filing is available 2417 from any computer with internet access. Go to the
NetFile Filing Portal at https://netfile.com/filer. Click the "New User? Request a Password" link in the "Form
700 (SEI) Filers" section below the log -in form. Enter and submit the e-mail address at which you received this
email. NetFile will send you an e-mail containing a link. Click the link to display your password. Make sure to
record the password as it is displayed. Go back to the NetFile Filing Portal (https://netfile.com/fifer) and log in
with your e-mail address and password. Once you log in, you may change your password by clicking the
"Change NetFile User Password" link in the bottom right menu in your filer account.
Important notes from NetFile:
After you log in, click the "Get Help for this Page" button for information and instructions. Each page also
includes links to short instructional videos.
At the end of the process, you can create a draft document for review. When you are satisfied with the
document, please e -file your document by clicking on the "E -File Statement" button. You do not have to print,

Two (2) Vacancies on the
Lodi Senior Citizens Commission
The City of Lodi is currently accepting applications for
the Lodi Senior Citizens Commission, developed
to identify the needs and issues specifically affecting the
senior citizens of the community and initiate action to
assure they are addressed.
The Lodi Senior Citizens Commission meets at 8:00
a.m. on the 1st Thursday of each month at the Senior
Center at Hutchins Street Square, and consists of 7
members, each serving 4 -year terms.
Applications are now being accepted to fill two (2)
vacancies on the Lodi Senior Citizens Commission for
4 -year terms, which will expire on December 31, 2025.
