RECOMMENDED ACTION: Appointment to the Measure L Citizens' Oversight Committee.
BACKGROUND INFORMATION. The City Council directed the City Clerk to post for two expired terms
on the Measure L Citizens' Oversight Committee on January 6, 2021,
March 17, 2021, and May 19, 2021. At the July 21, 2021 meeting,
City Council directed the City Clerk to fill the District 5 vacancy and to
repost for the District 4 vacancy for the period of one week.
Council Member Khan will review the applications and recommend an appointment, which will be
provided via Blue Sheet prior to the meeting.
FISCAL IMPACT: Not applicable.

MEASURE L CITIZENS' OVERSIGHT COMMITTEE - 5 members, 3 -year terms. Meets 3 times per year, Carnegie Forum.

Purpose: To review the annual accountability report and report its findings to City Council, in relation to revenues generated through
the general retail transaction and use tax approved by the citizens as Measure L.

public engagement.

In ending your service, it will be necessary for you to complete a Form 700 — Conflict of Interest Leaving Office
Statement no later than 30 days from the date of this letter of notice (due Friday, August 27, 2021). The
original statement will be filed with the Fair Political Practices Commission (FPPC). Instructions are included
below for filing online through NetFile.

1. Log in to the system at https://netfile.com/filer.

3. If you have lost or forgotten your password, click the "Lost Your Password?" link, enter your City email
address, and the system will send you an e-mail with a link to set your new password. Your old password will
become invalid.
4. Log in to begin the Form 700 filing process. After you log in, click the "Get Help for this Page" button for
information and instructions. Each page also includes links to short instructional videos. At the end of the
process, you can create a draft document for review. When you are satisfied with the document, please a -file

Congratulations! The City Council, at its regular meeting of August 4, 2021, has concurred with the Council
Member Khans recommendation to appoint you to the Measure L Citizens' Oversight Committee for a three-
year term (expiring June 30, 2024).
Please take this moment to read the attached Oath of Office, sign and date the form, and return to the City
Clerk's Office at your earliest convenience.
A copy of this letter will be forwarded to the City liaison for this group who will contact you soon regarding your
responsibilities and the dates and times of upcoming meetings.
Also attached is the City of Lodi's Commissioner's Handbook. It has been prepared to contribute to your
general knowledge and understanding of public affairs and to aid in identifying the scope and parameters of
your duties and responsibilities. While participation on a City commission is a major responsibility, we hope
that it will prove to be a meaningful and rewarding experience for you. City service provides opportunities to
gain a greater understanding of the issues facing municipal government and to become actively involved in
resolving those issues in a manner that reflects the best interest of the community.
As explained in the Handbook, as an appointed member to a City commission, it is important to comply with
the following requirements in order to remain in good standing as a member:
All commissioners must file an Oath of Office form with the City Clerk's Office within 30 days of
appointment (or reappointment) and prior to participating in any commission meetings or activities.
All commissioners must comply with AB1234 ethics training specified by Government Code section
53235 within 30 days of assuming office and thereafter by December 31 of even -numbered years.
(EXCEPTION: Greater Lodi Area Youth Commissioners — Student Appointees)
Commissioners must comply with Fair Political Practices Commission (FPPC) regulations pertaining to
Conflict of interest — Form 700s and are required to file an Assuming and Leaving Office Statement
within 30 days of assuming or leaving office and annually thereafter by April 1. (This requirement
pertains to Library Board of Trustees, Lodi Arts Commission, Lodi Improvement Committee, Measure L
Citizens' Oversight Committee, Parks and Recreation Commission, Planning Commission, and
SPARC.)
Ethics training is provided free of charge online by the FPPC at http://www.fppc.ca.gov/learn/public-officials-
and-employees-rules-lethics-training.htmi. Please choose the "Local Officials" training. Once your training
session is completed, you must print the Certification of Completion and provide a copy to the City
Clerk's Office. Please note — your certificate must indicate that training has been for a minimum of two
hours.
It will be necessary for you to complete a Form 700 — Statement of Economic Interest Assuming Office
Statement no later than 30 days from the effective date of your appointment (due Friday, September 3, 2021).
Please refer to the enclosed Resolution 2021-11 for disclosure category requirements regarding your particular
board, commission, or committee. The City of Lodi utilizes NetFile to electronically file Statement of Economic
Interest forms. Online filing is available 24/7 from any computer with internet access. Go to the NetFile Filing
Portal at https://netfile.com/filer. Click the "New User? Request a Password" link in the "Form 700 (SEI) Filers"
section below the log -in form. Enter and submit the e-mail address at which you received this email. NetFile
will send you an e-mail containing a link. Click the link to display your password. Make sure to record the
password as it is displayed. Go back to the NetFile Filing Portal (https://netfile.com/filef) and log in with your e-
mail address and password. Once you log in, you may change your password by clicking the "Change NetFile
User Password" link in the bottom right menu in your filer account.
Important notes from NetFile:
After you log in, click the "Get Help for this Page" button for information and instructions. Each page also
includes links to short instructional videos.
At the end of the process, you can create a draft document for review. When you are satisfied with the
