RECOMMENDED ACTION: Appoint Mitchell Slater and Steven Hennecke to the Planning
Commission and Frankie Kooger and Caitlin Casey to the Library
Board of Trustees.
BACKGROUND INFORMATION: The City Council directed the City Clerk to post for two expiring terms
on the Planning Commission on March 17, 2021. City Council also
directed the City Clerk to post for two expiring terms on the Library
Board of Trustees on March 17, 2021 and May 19, 2021.
The Mayor reviewed the applications, conducted interviews, and recommends that the City Council
concur with the appointment as detailed below.

Purpose: To review the annual accountability report and report its findings to City Council, in relation to revenues generated through
the general reti it transaction and use tax approved by the citizens as Measure L.

It will be necessary for you to complete a Form 700 — Statement of Economic Interest Assuming Office
Statement no later than 30 days from the effective date of your appointment (due Friday, July 2, 2021).
Please refer to the enclosed Resolution 2021-11 for disclosure category requirements regarding your particular
board, commission, or committee. The City of Lodi utilizes NetFile to electronically file Statement of Economic
Interest forms. Online filing is available 24/7 from any computer with internet access. Go to the NetFile Filing
Portal at httos://netfile.co-m/filer. Click the "New User? Request a Password" link in the "Form 700 (SEI) Filers"
section below the log -in form. Enter and submit the e-mail address at which you received this email. NetFile
will send you an e-mail containing a link. Click the link to display your password. Make sure to record the
password as it is displayed. Go back to the NetFile Filing Portal (https://netfile.com/filer) and log in with your e-
mail address and password. Once you log in, you may change your password by clicking the "Change NetFile
User Password" link in the bottom right menu in your filer account.
Important notes from NetFile:
After you log in, click the "Get Help for this Page" button for information and instructions. Each page also
includes links to short instructional videos.
At the end of the process, you can create a draft document for review. When you are satisfied with the
document, please a -file your document by clicking on the "E -File Statement" button. You do not have to print,
sign and submit a paper copy of your a -filed statement.
