BACKGROUND INFORMATION: The Mayor received a request from the Lodi Memorial Hospital Foundation for a letter of support for the Fit Life Impact Program grant application through the Community Foundation of San Joaquin's Community Health Grant Program. There was a need to send the letter immediately in order to make the application deadline. The Lodi Memorial Health Foundation is partnering with Lodi Unified School District, Lodi Memorial Hospital, and the Got Kids Foundation to develop a children's activity and health education program for grades 4, 5, and 6 to try to bend the obesity curve in the community. This joint venture will instill a fit lifestyle, develop career decisions, and reduce life-threatening obesity through nutrition, health, and career education coupled with physical activities and training. The healthy lifestyle choices learned during the 12 -week program will positively affect students and their families. The attached letter, electronically signed by the Mayor, was sent on September 22, 2017. A copy of the initial request is also attached. MAGDICH City Attorney RE: Fit Life Impact Program (FLIP) Grant Application - Letter of Support Dear Mr. Zapien: The City of Lodi is very excited to learn about and offer our support for the Fit Life Impact Program (FLIP) grant application submitted to the Community Foundation of San Joaquin through its Community Health Grant Program. The Lodi Memorial Health Foundation, which has partnered with Lodi Unified School District, Lodi Memorial Hospital, and the Got Kids Foundation, is developing a children's activity and health education program for grades 4, 5, and 6 to try to bend the obesity curve in our community. It is rewarding to see several of the largest institutions in our community and two impact foundations working together to combat one of the most significant healthcare crisis in our community and nation. No single organization could address this issue so comprehensively as this coalition, and the excellent leadership and track records of each of these partnering institutions leaves no doubt about what they can accomplish. Both the School District and Hospital have demonstrated their pledge to FLIP by committing staff time, expertise, and use of their facilities. This joint venture will instill a fit lifestyle, develop career decisions, and reduce life- threatening obesity through nutrition, health, and career education coupled with physical activities and training. The healthy lifestyle choices learned during the 12 -week program will positively affect students and their families. The City of Lodi is in full support of the FLIP grant application and program that will help improve and extend the lives of our children by teaching them healthy behaviors to prevent obesity. With adequate financial support, this program could and should expand throughout the County. FLIP — Fit Life Impact Program is a joint venture with Lodi Unified School District, Lodi Memorial Hospital, Got Kids Foundation and the Lodi Memorial Hospital Foundation, formed for the purpose of developing a comprehensive program to instill a fit lifestyle, develop career decisions and reduce life-threatening obesity through nutrition education, health education and career education coupled with physical activities and training. This program will serve some 100 students (400 family members) through 12 -week programs and monthly follow up events, offered at two schools for approximately 30 students throughout the school year. All organizations involved are registered 501c3 corporations. Lodi Unified School District [Lead Organization] Attn: Cathy Nichols -Washer, Superintendent 1305 E. Program will commence February 12, 2018 and run through December, 20 2018. Grant Request: The grant request is for full funding at $50,000 for the initial year which will free up other funding sources to expand the program in the initial year. 1 ORGANIZATION DESCRIPTION Primary Organization: The Lodi Unified School District, a 501c3 California corporation, formed in 1967 and serves 29,800 students in grades K-12 within its 350 square mile district zone. The district employed 2,291 full-time employees and 921 part-time employees in 2017. The annual operating budget exceeds $320M and is supported through state funding and bonds. In addition to classroom education, the school district offers enhanced academic programs and a complete interscholastic athlete program and operates a formal "After -School Program" for students on an elective basis. LUSD is governed by a seven -member Board of Directors whose current President is Ron Heberle. A complete list of Directors follows. LUSD will operate the program in partnership with Adventist Health — Lodi Memorial Hospital and received financial support from Got Kids Foundation and Lodi Memorial Hospital Foundation. Fairmont Ave. Suite 3 Lodi, CA 95240 209-339-7833 wcraig@lmhfoundation.org LMHF will provide financial support of FLIP and program coordination with Lodi Memorial Hospital. 2 PROJECT NARRATIVE Program Rationale: Studies have shown a quadrupling of childhood obesity over the last 40 years to 19 percent, and recent research estimates that 1 in 2 Americans have diabetes or pre -diabetes. San Joaquin County has some of the highest rates of diabetes and obesity in California. The Heritage District (eastern section) of Lodi, California is disproportionately affected by these poor health statistics and socio-economic challenges, including 38 percent poverty. Children are most adversely affected by these chronic diseases and socio-economic indicators. Research has even shown children diagnosed with diabetes have decreased life expectancy by approximately 19 years. Our community must address these health outcome challenges. Project Goals: Master Goal: Reduce obesity and develop healthy lifestyle adoption within the target population. Objectives: 1. Healthy Lifestyle Adoption 2. Healthy Nutrition Choices 3. Health Management (education & self-management) 4. Healthcare Career Education Grantor Goal Alignment: The program goals align directly with the Grantor goals in areas of health education, healthcare workplace development and healthy outcomes. Target Population: Lodi Heritage District Schools-4th,5tn & 6th Grade Elementary Students and their families Approximately 100 families (400-500 people) from school located in the highest impact area. Program Elements: This will be a 12 -week course taught in the After School Program and will run at 2 elementary schools throughout the school year plus monthly follow-up events and continued data tracking. Additionally, the program will begin with a Kick -Off event involving parents and students and will conclude with a Graduation Event also including parents. The program will be coordinated by the LUSD After School Program Coordinator and will engage professional trainers, certified teachers and experts in the field as well as professionally licensed dietitians and healthcare staff. 3 1. Healthy Lifestyle (Activity Component): Twice weekly after-school exercise programs exposing a variety of fitness approaches. Professional trainers in each field with work with the Project Director and Physical Education teachers will expose students to a variety of ways to develop a lifestyle of fitness. A. Zumba for cardio B. Yoga for breathing C. Run -walk programs for endurance D. Resistance Training for strength E. Cross -Training for overall conditioning 2. Nutrition Education Students will receive presentations and activities on the importance of healthy food choices for health. A. Nutrition Education from certified dietitians B. Food preparation activities (students will cook for their parents) C. Personal weekly menu creation D. Intake logging 3. Health Management Students will visit Lodi Memorial Hospital for an In -Service experience to learn about health issues and the current state of medicine with the goal of making healthy choices the highest priority. A. Students will tour departments at Lodi Memorial Hospital to learn what happens when health fails. B. Students will visit with Physical Therapy, Food Service, Emergency Department and meet with nurses and a physician to hear details about the role lifestyle plays in health C. Students will be required to produce a report on their experience and findings 4. Healthcare Career Education As a component to healthy living and healthcare education students will introduced to career opportunities in the healthcare industry. A. Students will visit Lodi Memorial Hospital and hear from various departments about how to engage a career in healthcare. Lodi Memorial Hospital is the largest employers in Lodi and has ongoing career opportunities for students to pursue. B. Student will learn the importance of academic education and healthy lifestyle in developing a career. Program Management: The program will be coordinated by the After School Program Coordinator of LUSD (at no charge) in conjunction with credentialed Physical Education Teachers, professional physical trainers/instructors, volunteers and linked to regular PE classes taught at the school as well as special session, field trips and internships. 4 Measurement of Outcomes: The FLIP program will be measured through a variety of tests and tools. Data will be reviewed by educational and healthcare professionals and compared to State of California Physical Fitness Standards. A. A pre-course/post-course matrix will be created to illustrate changes resulting from program participation in areas of knowledge and fitness. B. Data Tracking- students will maintain logs of activities which will be used to compare with the State of California Physical Fitness Standard tests reported annually for 5th grades. Categories will include Aerobic Capacity, Body Composition, Abdominal Strength, Trunk Extension Strength, Upper Body Strength and Flexibility. Note: The program will attempt to acquire Fitbit Monitors for each student to gather and record activity data. C. Subject to parental approve, medical screening of Body Mass Index, cholesterol level and vital signs will be measured and recorded at the beginning, periodically and at the end of the program window for changes in condition. Use of Evaluations and Measurements: Data collected will be used to create performance charts to understand the effectiveness of the program make modifications for future classes. Students will be encouraged to continue the activity routine on their own after the initial program period. Parents will be provided with the data results and encouraged to support healthy lifestyles for their children and self.
