HomeMy WebLinkAboutAgenda Report - September 6, 2017 C-08CITY OF LODI COUNCIL COMMUNICATION TM AGENDA ITEM C - 8 AGENDA TITLE: Adopt Resolution Authorizing the City Manager to Execute a Five -Year Professional Services Agreement with Adventist Health Lodi Memorial for Annual Physical Exams for the Lodi Fire Department (Not to Exceed ($17,000 Annually) MEETING DATE: PREPARED BY: September 6, 2017 Fire Chief RECOMMENDED ACTION: Adopt resolution authorizing City Manager to execute a five-year Professional Services Agreement with Adventist Health Lodi Memorial for annual physical exams for the Lodi Fire Department (not to exceed $17,000 annually) BACKGROUND INFORMATION: In 2013, the City of Lodi Fire Department entered into an agreement with Adventist Health Lodi Memorial to complete annual physicals for the Lodi Fire Department's staff, which is a requirement under the Lodi Professional Firefighters Memorandum of Understanding. In anticipation of the term ending with the existing service agreement, staff distributed a Request for Proposals to three firms within the City of Lodi on February 6, 2017. Out of the firms who received the RFP, only Adventist Health Lodi Memorial responded. The response was reviewed by staff based on the understanding and approach to the scope of work, firm qualifications and experience, quality and present workload, and staff availability. Based on this review, it was determined by staff that Adventist Health Lodi Memorial was the best value for the City and the agency most capable of meeting the Fire Department's needs. Adventist Health Lodi Memorial is located in Lodi and provides the City's occupational health services. By contracting with Adventist Health Lodi Memorial, expenses for annual firefighter physicals will not increase over the next five years based on the attached fee schedule. Staff recommends authorizing the City Manager to enter into a five-year Professional Services Agreement with Adventist Health Lodi Memorial. FISCAL IMPACT: Based upon historical usage, it is anticipated that annual costs will be estimated at $17,000 a year for the next five years. FUNDING AVAILABLE: This project cost is included in the Fiscal Year 2017/18 Emergency Operation budget (10041000.72373) APPROVED: chwab - , City Manager Andrew Keys Deputy City Manager/Internal Services Director ooney Fire Chief AGREEMENT FOR PROFESSIONAL SERVICES ARTICLE 1 PARTIES AND PURPOSE Section 1.1 Parties THIS AGREEMENT is entered into on November 1, 2017, by and between the CITY OF LODI, a municipal corporation (hereinafter "CITY"), and ADVENTIST HEALTH LODI MEMORIAL (hereinafter "CONTRACTOR").
