AGENDA TITLE: Appointment to the Lodi Arts Commission and the Lodi Senior Citizens Commission

RECOMMENDED ACTION: Concur with the Mayor's recommended appointment to the Lodi Arts
Commission and the Lodi Senior Citizens Commission.
BACKGROUND INFORMATION: On various dates, the City Council directed the City Clerk to post for
vacancies on the Lodi Arts Commission and the Lodi Senior
Citizens Commission. The Mayor reviewed the applications,
conducted interviews, and recommends that the City Council concur with the following appointments.

of a vacated three-year term (expiring July 1, 2017).

return to the City Clerk's Office at your earliest convenience.
A copy of this letter will be forwarded to the City liaison for this group who will contact you
soon regarding your responsibilities and the dates and times of upcoming meetings.
It will be necessary for you to complete a Form 700 — Statement of Economic Interest
Assuming Office Statement (enclosed) no later than 30 days from the effective date of your
appointment (due Monday, February 8, 2016, by 5:30 p.m.). Please refer to the enclosed
Resolution 2014-212 for disclosure category requirements regarding your particular board,
commission, or committee. The original statement will be retained by the City Clerk's office.
Please feel free to give me a call should you have any further questions and/or concerns
