BACKGROUND INFORMATION: On various dates, the City Council directed the City Clerk to post for vacancies on the Lodi Arts Commission and the Lodi Senior Citizens Commission. The Mayor reviewed the applications, conducted interviews, and recommends that the City Council concur with the following appointments. APPOINTMENT: Lodi Arts Commission Edward Casillas Steven Orr Term to expire July 1, 2017 Term to expire July 1, 2018 NOTE: Two applicants (no applications seeking reappointment, no applications on file, two new applications); posting ordered 7/15/15 and 8/5/15; application deadline — open until filled. Lodi Senior Citizens Commission Teresa Whitmire Term to expire December 31, 2019 Tracy Williams Term to expire December 31, 2019 NOTE: Three applicants (Two applications seeking reappointment, one application on file, no new applications); posting ordered 10/21/15; application deadline 11/30/15. It will be necessary for you to complete a Form 700 — Statement of Economic Interest Assuming Office Statement (enclosed) no later than 30 days from the effective date of your appointment (due Monday, February 8, 2016, by 5:30 p.m.). Please refer to the enclosed Resolution 2014-212 for disclosure category requirements regarding your particular board, commission, or committee.
