WHEREAS, as pursuant to Resolution No. 2669, the City of Lodi
heretofore granted an Ambulance Operators Permit to Philip L. Jolly,
Lloyd Helwig. and Melvin Helwig, co-partners doing business as Lodi
Ambulance Service;
WHEREAS, said Permittee has filed its written request with City of
Lodi to transfer said permit to Michael N. Nilssen and Dennis M. Cerruti:
and
WHEREAS, notice was duly given by publication, a public hearing
was held and no one appeared to object thereto:
NOW, THEREFORE, the City Council of the City of Lodi does hereby
find as follows:
(1) That affidavit of publication showing that this matter was
regularly set for public hearing is on file in the office of the
City Clerk.
(2) That the aansferee will continue to use the same color scheme.
insignia and name as was used by the present Permittee.
(3) That a continued ambulance service in the City of Lodi is required
by the public convenience and necessity and that the new applicants
are fit, willing and able to perform ambulance service and to
conform to the provisions of Chapter 2-A of the Code of the City
of Lodi and to rules and regulations as may be promulgated by
the City Council of the City of Lodi.
WHEREAS, the present Permittees have filed their written declaration
that upon the granting of permission to transfer the existing permit they will
simultaneously surrender the present permit and concurrently a new permit

would be issued by the City of Lodi to Michael N. Nilssen and Dennis M.
Cerruti, co-partners doing business as Lodi Ambulance Service.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the
City of Lodi, that pursuant to the foregoing findings:
(1) The permit heretofore issued to Philip L. Jolly, Lloyd Helwig
and Melvin Helwig, co-partners, doing business as Lodi Ambulance
Service, is hereby canceled.
(2) That a permit is hereby granted to Michael N. Nilssen and
Dennis M. Cerruti, co-partners doing business as Lodi Ambulance
Service, whose address is 340 South Fairmont Avenue, Lodi,
California, for the operation of an ambulance service within the
City limits of the City of Lodi.
(3) The foregoing permit is granted subject to said operators complying
with all of the conditions and provisions of ChaP_fer.2aA of the Code
of the City of Lodi for the operation of the ambulance service
described in said application with the equipment from the place
of business and the rates established and set forth in their appli-
cation to the City Council of the City of Lodi dated November 10,
1971.
BE IT FURTHER RESOLVED that the effective date of this permit is
December 1, 1971.

by the City Council of the City of Lodi in regular meeting held
December 1, 1971 by the following vote:
