RESOLUTION GRANTING SUPPLEMENTAL BENEFITS
TO EMPLOYEES AS A RESULT OF WITHDRAWING
FROM SOCIAL SECURITY
RESOLVED, by the City Council of the City of Lodi,
effective July 1, 1977 Uniform Allowance will be paid to the following
classifications as designated below:
Dispatcher/Matron $70/per Quarter
Dispatcher/Jailer $70/per Quarter
Police Technician $70/per Quarter
Fire Dispatcher $50/per Quarter
Parking Enforcement Assistant $50/per Quarter
Animal Control Officer $60/per Quarter
Assistant Animal Control Officer $60/per Quarter
BE IT FURTHER RESOLVED by the City Council of the
City of Lodi that Resolution 4134 is hereby rescinded and that effective
July 1, 1977, payment of medical insurance premiums for all employees,
not to exceed $40.84, is hereby authorized.
BE IT FURTHER RESOLVED that the City Council of the City
of Lodi does hereby authorize payment of contribution by the City of Lodi
for dependent coverage as follows effective July I, 1977:
a) Management employees $54. 51
b) General Service & IBEW $35.57
c) Police & Midmanagement $42.98
d) Fire $38. 16

BE IT FURTHER RESOLVED that the City Council of the
City of Lodi does hereby authorize the following benefits in addition
to the modification of the City of Lodi's contract with the Board of
Administration Public Employees' Retirement System to partially re-
place coverage lost through withdrawal of Social Security for all but
Safety employees.
a) The City agrees to match an employee's 1%
contribution to deferred compensation
b) The City agrees to provide a long-term
disability plan providing a benefit of 66-Z/3%
of an employee's salary in the event of disa-
bility. This benefit to be coordinated with
other disability benefits and to commence 60
days from disability or at the exhaustion of
sick leave benefits, whichever is greater.
The cost of this program will be 1.4% of
salary.
c) The City agrees to provide an additional
$10, 000 term life insurance for each em..
ployee and $1, 500 for each dependent child.
The cost of this program will be no more
than $6.68 per employee.
The long term disability insurance and life insurance will be
written through Standard Insurance Company. Each employee will
make a mandatory contribution of I. I% of base salary as their share
of the cost of this program.

I hereby certify that Resolution No. 4367 was passed
and adopted by the City Council of the City of Lodi in
an adjourned regular meeting held June 28, 1977 by
the following vote:
Ayes: Councilmen - Ehrhardt, Hughes, Katzakian,
Pinkerton and Katnich
Noes: Councilmen - None
Absent: Councilmen - None
