FERNANDEZ AND THE CITY OF LODI RESOLVED, that the Lodi City Council hereby approves the amended contract, Exhibit A attached hereto, between Paula J. Fernandez and the City of Lodi, granting a promotion and a title change to that of Associate Traffic Engineer. Rei the City Clerk &�I RES9295/TXTA.02J CITY OF LODI PERSONAL SERVICES CONTRAC` Paula J. Fernandez and City of Lodi This is to certify that I, Paula J. Fernandez, continue employment with the City of Lodi .in the position of Associate Traffic Engineer at a present base salary of $3,564.09 per month (80 hours per two-week pay period) on a regular, full-time basis, and wish to work half-time (job share). It is understood that it is my intention to work 40 hours each 80 -hour pay period. It is further understood that I will retain all the rights and privileges of a regular employee as covered by the Memorandum of Understanding (MOU) for the General Services Unit except as described below: 1. Salary will be prorated to hours worked per pay period. 2. Administrative Leave will be 20 hours per year. 3. Vacation Leave accrual will be 50% of that in the MOU. 4. Sick Leave accrual will be 50% of that in the MOU. 5. Holiday Leave will be 50% of that in the MOU. 6. Medical and dental insurance coverage will not be provided, however, they may be reinstated upon employee's contribution of 50% of the current premium and with evidence of insurability or upon return to full-time status. 7. Long-term disability benefit to be 50% of that provided in the MOU. 8. The City will pay both the employer and employee contributions to PERS. This agreement may be terminated under the following circumstances: 1. With 60 days written notice of the City's desire to fill the Associate Traffic Engineer position on a full-time basis provided the City has made a good faith effort to find another person to fill the other half of the job -share position. 2. With 30 days written notice of the employee's wish to return to full-time status, provided the other half-time position is not filled. 3.
