Shively regarding his health care option(s) with the City. A July 27, 1990 City Council City of Lodi Lodi, California Members of the City Council: 4 After my disability retirement in April, 1980, I has built tip five years and 2 months of converted sick leave to May 31, 1983. On June 1, 1985, I was allowed to convert my City of Lodi Group Plan to Foundation Health at my own expense, but under the group rate, because of the kindness and generosity of the City Council. Because I had Medicare. Insurance due to a disability retirement, I was given the supplemental plan offered by Foundation Health which also paid for perscription drugs. My wife, Marilyn, was carried as a dependent. The total premium per month was 5205.30. On May 25, 1990, I received notification from the City that the last date I would be covered under the City of Lodi's group rate would be July 31, 1990. I immediately went to the Foundation Health office in an effort to effect a smooth transition to an i nd iv idu al pol icy . Upon learning that I was not 65 years of age, they, although somewhat concerned, felt arrangements could be made to continue the same benefits for my wife and myself at nearly the same premium as we had been paying. On June 4, 1990, Foundation health wrote me a letter thanking me for my recent application for Foundation 65, but gelling me that. because i was not 65 years of age, I could not. have the supplemental coverage, even though I have Medicare co\erage. I could not believe that I needed anything more than supplemental coverage, so I attempted to speak with anyone who could giye me a reasonable answer from the second week in June until mid-July-, and finally reached a person who evidently hat: t.hc• f i li a.l authority. I have come before the City Council before to ask that the right thing be done, and the Council, in its wisdom and compassion, has granted my request. I am asking once again that you review this situation and grant that I may, at my own expense, continue to pay the total premium on a health care plan. I see no good reason why I have to be removed from the City of Lodi Group Health Plan, and hereby request to be allowed to remain in that Group Plan until my sixty- fifth birthday four years from now. There is a thirty day grace period after my present coverage terminates on July 31, 1990. It would be very beneficial for me if I could be reinstated before that time is itp.
