BACKGROUND INFORMATION: The City Clerk's Office received a letter of resignation from Lodi
Animal Advisory Commissioner Daniel Crownover (filed).
Additionally, the most recent appointee to the Site Plan and
Architectural Review Committee chose not to accept the appointment at this time. Government Code
Section 54970 et sea. requires that the City Clerk post for vacancies to allow citizens interested in
serving to submit an application. Therefore, it is recommendedthat the City Council directthe City Clerk
to post for the vacancies shown below.

Site Plan and Architectural Review Committee
Keith Selleseth Term to expire January 1, 2012

dedicated service as a member of the Lodi Animal Advisory Commission.
As you reflect on your participation, I hope you will feel gratified in knowing you have been a
part of many fine events and accomplishments and that effort will continue to be made toward
enriching and improving our community. Thank you so much for sharing your talents with us.
In ending your service, it will be necessary for you to complete a Form 700 — Conflict of
Interest Leaving Office Statement no later than 30 days from the date of this letter of notice
(due Monday, August 29,2011, by 5;30 p.m.). The original statementwill be retained in the
City Clerk's Office. Forms have been enclosed for your convenience.

Lodi Animal Advisory Commission
One vacancy - term to expire December 31, 2013 (2-yearterm)
Purpose: To act in advisory capacity to City Council in all matters pertaining to
animals; explore short- and long-term needs of the Lodi Animal Shelter;
make periodic inventories of animal services; aid in coordinating and
promoting pet population control measures and explore funding
opportunities for programs; and serve as an information and referral
source on animals in the community.
Meetinas: Meets2nd Mondayof each month at6:00 p.m.
Lodi Police Department- Community Room
Membership: Seven members
Site Plan and Architectural Review Committee (SPARC)
One vacancy - term to expire January 1,2012 (1 -year term)
Purpose: To assist the Planning Commission in reviewing site plans and
architectural drawings in order to determine compliancewith zoning
ordinances and to promote the orderly development of the City.
Meetinas: Meets 2nd and 4t" Wednesdays of each month at 5:15 p.m.
Carnegie Forum
Membership: Five members
Note: Appointments will be made by the Mayorwith Council approval. Applicants
must be registered to vote in San Joaquin County (or City of Lodi for SPARC) at
the time of application. If appointed, you may be required to complete a
Statementof Economic Interest. These are volunteer, unpaid positions.
Applications may be obtained from the City Clerk's office, 221 West Pine Street,
