AGENDA TITLE: Resolution of the Lodi City Council Approving a Grant Award for the
City of Lodi Smoking Ordinance Pron'totion Project
MEETING DUE: January 15, 1992

RECOMMENDED ACTION: That City Council adopt the attached resolution.
BACKGROUND INFORHATION: On October 28, 1991, staff submitted an application
for $8,483 i n gract funding from California Smoke -Free
Cities. The grant was awarded December 11, 1991.
This grant w i I I pay for staff time and the printing of
brochures/pamphlets/posters to inform the public about
the City's smoking ordinance and encourage children and adults not to smoke.
The attached resolution is required by California Smoke -Free Cities to release
funds.
FUNDING: None required.

A RESOLUTIOM OF THE LODI CITY COUNCIL APPROVING A GRANT
A�IARO FOR THE CITY OF LOO € SMOKING ORDINANCE PROMOTION PROJECT.
WHEREAS, the California OQQartment of Health Services is the first state
health agency to sponsor a Healthy Cities Project; and
WHEREAS, the California Healthy Cities Project has joined with the
League of California Cities, Americans for Nonsmokers' Rights. and the Health
Officers Association of California in an affiliated cQmmunity improvement
project, California Smoke -Free Cities; and
WHEREAS. California Smoke -Free Cities has awarded the City of Lodi
funding in the amount of 58,489, made possible by Proposition 99, the 1988
Tobacco Tax Initiative;
NOW, THEREFORE. BE IT RESOLVEO, that the City Council of the City of
Lodi authorizes the awarding of S8,489 for the purpose of implementing the
City of Lodi Smoking Ordinance Promotion Project. The City Manager of the
City of Lodi is hereby authorized and empowered to execute in the name of the
City of Lodi all necessary agreements and amendmenrs to secure grant funds
and to implement and carry out the purposes specified in the Smoking
Ordinance Promotion Project outline.
The foregoing resolution was passed by the City Council of the City of
Lodi this 15th day of January, 1992 and is effective January 15, 1992.

The Western Consortium for Public Health, California Smoke -Free Cities. hereinafter WCP1I/CSFC: afid
the City of Lodi, hereinafter CITY, agree as follow:,. effective on the subscribed date:
PURPOSE OF GRANT: City agrees to use the funds provided by this grant exclusively for the purpose
of conducting the project activities described in its California Smoke -Free Cities Mini -Grant application.

TERM: The term of this grant shall be from the effective date through the date of completion of the
project or September 30, 1992,whichever is earlier.
PAYMENT: Upon receipt of a fully -executed grant agreemect from CITY. WCPH/CSFC shall pay CITY
58,489. Project expenditures shall be in accordance with the attached Exhibit A (Budget). City may
modify the budget with prior notification to WCPH/CSFC.
PROGRESS AND FISCAL REPORTING: CITY shall provide an interim progress report on March 31,
1992 and a final progress report on or before September 30. 1992. The final report shall include all
evaluation of the project's impact on the community. CITY shall provide a final fiscal report detailing its
expenditures on or before November 30, I992. CITY shall promptly return the unexpended balance, if
any, of funds awarded under this grant agreement.
AUDIT: CITY agrees to maintain and preserve until December 31, 1995; and to permit WCPII/CSFC
and the State or any of its duly authorized representatives to have access to and examine and audit, any
pertinent books, documents, papers, aril records of CITY.
ACKNOWLEDGEMENT: CITY shall acknowledge the financial support of Proposition 99 funds in any
publications or materials produced with grant support as follows: 'This was made possible by
funds received from Proposition 99, the 1988 Tobacco Tax Initiative.
STATE USE OF MATERIALS: The State of California shall have the right to use, reproduce, and
authorize others to use and reproduce, any materials produced pursuant to this grant.
